Carer's Assessments

All Carers are entitled by law to receive a Carer's Assessment.   Carers of West Lothian encourages all carers to ask for an assessment and believes that this is a useful step that will help the statutory authorities to become more aware of carers' needs.

The two documents below have been published jointly by Carers of West Lothian, NHS Lothian and West Lothian Council.  The first leaflet gives more information about Carer's Assessments and the second includes a form which carers can complete and send to their local Social Work Centre to start off the process.

The actual Carers Assessment Form is a lengthy one and we make it available here, along with Guidance Notes for Professionals on its completion, so that carers can download it and thus be fully aware of what is involved.
