Carer Support Worker

(Pre-hospital discharge)

Two-year fixed term contract
28/35 hours per week (35 hours subject to final agreement with NHS Lothian)
Salary £21,861 - £24,201 pa pro rata

Based within St John’s Hospital in Livingston and the Edinburgh Royal Infirmary, the post holder will identify and support unpaid carers to access appropriate support as part of the discharge planning process of a relative, family member or friend.

The successful candidate will have a good understanding of caring issues and carers’ needs, experience of providing support to individuals, a good understanding of care in the community, and a professional qualification relevant to the post.

Project Background

This national pilot supports unpaid carers to plan for hospital discharge and home based support of a relative, family member or friend. It aims to avoid crisis and readmission, and assists long term condition management and self-care support.

The project is funded by The Moffat Charitable Trust through The Princess Royal Trust for Carers and supported by NHS Lothian, City of Edinburgh Council and local carer centres.
