Carer Support Worker(Pre-hospital discharge)Two-year fixed term contract Based within St John’s Hospital in Livingston and the Edinburgh Royal Infirmary, the post holder will identify and support unpaid carers to access appropriate support as part of the discharge planning process of a relative, family member or friend. The successful candidate will have a good understanding of caring issues and carers’ needs, experience of providing support to individuals, a good understanding of care in the community, and a professional qualification relevant to the post.
